Once your appointment is scheduled, you'll receive your intake paperwork approximately two weeks before your appointment.

Please complete and submit your intake forms at least 48 hours before your scheduled appointment. This gives me time to review everything you've shared before we meet so we can spend our appointment focused on understanding you rather than completing paperwork.
